While Toronto is in step three of the province’s roadmap to reopening, we ask that you adhere to the following health and safety guidelines leading up to and during your visit.

If you’re experiencing symptoms or believe you’ve been in contact with someone who has tested positive for COVID-19, please contact us to cancel your appointment. We encourage you to contact your local health unit for further guidance.

Please review, complete and submit the patient health screening form, which will be emailed to you prior to your appointment.

For your safety, we’re staggering appointments. Please call or text (647) 768-2586 when you arrive in the parking lot and we’ll let you know when you may enter the building.

We also suggest arriving as close to your appointment time as possible, as our waiting rooms are currently closed.
